How It works:

  2. Print the form. You will be redirected to a webpage with your form submission, and a copy will be emailed to you
    *** If you are not redirected, please refresh the page and your completed form should display.
    You can also print the form sent to your email. Print the form, and include it with the encoder you're sending in.

  3. We will contact you with a repair estimate after your encoder has been evaluated.
NOTE: To help us identify your unit quickly, please include a printed copy of this form when shipping your encoder.

  • No repairs are started without your approval.

  • Evaluation or repair fees may apply, and customers are responsible for shipping costs.
